Substance Misuse Support – East Riding

What we do

This service provides structured community support for people in the East Riding with drug and/or alcohol issues. This is a collaboration between Humber Teaching NHS Foundation Trust Alcohol and Drug Service and Nacro East Riding Partnership.

The Nacro Substance Misuse team support people who are leaving police custody, prison, or court, and includes those referred by Probation or who are self‑referring.

We support those who have found themselves in the criminal justice system and who require resettlement, harm reduction, and support to maintain long‑term positive change.

We support people who:

  • Use drugs and/or alcohol.
  • May require opiate substitute medication.
  • Are abstinent but needing ongoing support.
  • May require anti-craving medications for alcohol.
  • Have been ordered by the Court to address their drug and/or alcohol use.
  • Have specific licence conditions.

Our Recovery Practitioners offer extensive recovery expertise. The team deliver interventions using CBT, motivational interviewing, person‑centred approaches, and some draw on lived experience.

Support provided:

  • Comprehensive assessment and personalised care planning.
  • Intensive one‑to‑one support with an emphasis on building strengths based recovery.
  • Access to prescribing and referrals to wraparound services.
  • Smooth transition into East Riding Partnership community services for ongoing support where needed.
  • Relapse prevention and overdose awareness.
  • Identifying triggers, risky situations, and safer responses.
  • Challenging unhelpful beliefs and thinking patterns to support behaviour change.
  • Pre‑arrest and custody‑based DIVERT interventions.
  • Alternatives to prosecution or cautions for low‑level or first‑time offenders.
  • Access to educational courses and substance misuse treatment options.

Our IOM team works closely with police and probation services, providing a cross‑agency response to crime and prolific reoffending risks within the local community.

 

Before release from prison, and to ensure a smooth transition from custody to community we offer:

  • Face‑to‑face, phone, and online (Teams) sessions.
  • Pre‑release visits, video links, and meet‑and‑greets.
  • Email‑a‑prisoner contact.
  • A full package of support on release.
  • Joint work with probation, benefits, and housing services.

For people who are hard to reach or have complex needs, we offer enhanced support throughout the East Riding which may include:

  • Joint appointments with substance misuse keyworkers and partner agencies.
  • Support in professional meetings.
  • Police and mental health liaison.
  • Housing and homelessness support.
  • Employment and training agencies.
  • GP or hospital appointments.
  • Help with benefits or housing/other applications.
  • Advocacy.

Referral information

We accept referrals from the police (Liaison & Diversion and DIVERT), probation, prisons, courts, out‑of‑area services, self‑referrals, and from our three support hubs.

I was supported with my alcohol misuse by Nacro. My keyworker helped with the goals I wanted to achieve, and I felt listened to. After leaving the service, I'm doing much better, and have got a part-time job. I'm still meeting with my counsellor and and am planning to move house with my partner soon.
