What we do
We support people with their resettlement needs from the day of release. This includes working with other organisations such as housing providers and mental health teams. We also support people with welfare benefits, registering with a GP practice, and opening bank accounts.
We assess a person’s needs before they are released from custody and work with agencies to ensure both a smooth transition to their community and a joined-up approach between healthcare and statutory resettlement.
People can be met at the gate upon release and escorted to all of their crucial appointments on the day, such as probation and local authority housing. We also make sure they have all the essentials, such as their correct medication, prescriptions and planned appointments for the first few days.
Support continues for 6-12 weeks to ensure the person gets appropriate safe and secure accommodation, access to welfare benefits, re-engages with health services, and strengthens links with family and community support services.
To be eligible a person must have been sentenced and being released into a London borough. They must have had a clinical diagnosis of a mental health condition and/or be on certain types of prescribed medication.
