A Place of Fear? Mental Health Act Commission's 11th Biennial report

Issue date: Jan 2006

This report by the Mental Health Act Commission said there was intense pressure on beds, understaffing and a lack of basic humane treatment. The report was based on visits to detained psychiatric patients in all hospitals in England and Wales between 2003 and 2005

The report also acknowledges the Nacro survey of court diversion schemes and repeats Nacro's recommendations to increase the number of psychiatric beds available and to adequately fund court diversion/criminal justice mental health liaison schemes across the country.

The London Prison Mental Health Forum

Forensic Psychiatric Nursing

Inspection of mental health services - Lambeth

Issue date: Jan 2005

This inspection by the Commission for Social Care found that mental health services for people in Lambeth were good. Many service users said they found the services too geared towards those with severe mental illness, forensic histories or a dual diagnosis and that primary care was not so good. A review by SLaM showed that the demand for forensic services had increased, especially from prison referrals. The effect of this was to place a premium on gatekeeping and to target areas of need e.g. young black men. There was also a lack of move-on accomodation. There was no mention of court diversion in the report.

The National Service Framework for Mental Health: five years on

Independent External Review into a Homicide at Prestatyn, Wales on 25th March 2003

On 25th March 2003, PK, a Cardiff resident, killed BD, a retired accountant. At the time of the offence PK was under the care of the Community Forensic Psychiatric Team at Whitchurch Hospital, Cardiff and was living in independent accommodation.

Celebrating our cultures: Guidelines for mental health promotion with BME communities

David Bennett Inquiry

Issue date: Dec 2003

Following the death of David Bennett in the Norvic Clinic, Norwich on 30 October 1998, an independent inquiry was set up to look into his care, treatment, the events surrounding his death and wider mental health issues.

The Independent Inquiry was set up under Health Service Guidelines HSG(94)27 by the Norfolk, Suffolk and Cambridgeshire Strategic Health Authority after consultation with the Department of Health. Mr Bennett, a 38 year-old African-Caribbean patient, died in The Norvic Clinic, a medium secure psychiatric unit in Norwich, at the end of October 1998 after being restrained by staff.

The Inquiry was chaired by Sir John Blofeld, a retired high court judge.

Dual Diagnosis Good Practice Guide: Mental Health Policy Implementation Guide

Supporting someone with a mental health illness and substance misuse problems - alcohol and/or drugs - is one of the biggest challenges facing frontline mental health services. The complexity of issues makes diagnosis, care and treatment more difficult, with service users being at higher risk of relapse, readmission to hospital and suicide. One of the main difficulties is that there are a number of agencies involved in a person's care - mental health services and specialist rehabilitation services, organisations in the statutory and voluntary sector. As a result care can be fragmented and people can fall down the cracks. The Department of Health guidance provides a framework within which staff can strengthen services so that they have the skills and organisation to tackle this demanding area of work. The authors key message is that substance misuse is already part of mainstream mental health services and this is the right place for skills and services to be. Mental health services must also work closely with specialist substance misuse services to ensure that care is well co-ordinated. The authors have also highlighted some existing examples ofexcellent services working well now. This guidance and the services mentioned, demonstrate the importance of effective leadership at a local level and rigorous training programmes to help staff maintain high standards of service delivery.

Outcome of psychiatric admission through the courts

Prison Health is a partnership between the Prison Service and the Department of Health working to improve the standard of health care in prisons. Their aim is to provide prisoners with access to the same quality and range of health care services as the general public receives from the NHS. In September 2002, Ministers announced the decision to transfer the budgetary responsibility for prison health from the Prison Service to the Department of Health. In addition a timetable of April 2006 has now been agreed for full devolution of commissioning responsibility to Primary Care Trusts (PCTs).

Report of the independent inquiry into the care and treatment afforded to Dale Pick
An independent inquiry into the care of Dale Pick said he was discharged against a background of bed shortages at the Leicestershire partnership NHS trust. The inquiry report reveals a catalogue of errors in Pick's care and treatment.

Pick's final hospital admission followed fears about him carrying a knife and hearing voices that told him to hurt others. Pick discharged himself from hospital just days before stabbing a man to death.
